Directions

We are currently located in the 10 Jackson Street Building in Los Gatos. It is suite 109 which is downstairs and in the middle of the courtyard.

More complex cases may be requested to also bring Metabolic Assessment form to the right.

Please bring copies of any recent, pertinent lab-work and the supplements and medications you are currently taking to your first visit.

Please ask Dr. Brad or staff if these extra forms are necessary if you think you are a complex case.
